AMM to Serve as Clearinghouse for Sample Documents
The Association of Midwest Museums (AMM) is collecting sample documents from museums and cultural institutions and will keep the documents on file, making copies available to AMM members who are searching for templates when developing forms for their institutions.

Museums are encouraged to submit sample documents might include a diversity statement, collections policy, a condition report, accession and de-accession forms, and job descriptions. Please consider contributing sample forms or documents from the following departments:
